Our mission is to store, protect and deliver accurate medical information to authorized users to support Memorial Hospital. - -Reviewing a Medical RecordYou may review your or your children's protected health information in the HIM department during business hours. We request you call 24 hours in advance to ensure the records are available upon your arrival. - - Obtaining a Copy of a Medical RecordYou may also request a copy of medical records. However, Memorial Hospital is required by law to obtain your written permission before releasing copies. If you desire to receive a copy of medical records, please print, complete and deliver an Authorization to Disclose Protected Health Information form to one of our locations.
