The Memorial Health System Pulmonary Case Management Program is designed by nurses with advanced training in respiratory care to answer your questions, monitor your condition and provide education.
Studies show that people with conditions, such as lung problems, who are closely monitored by health care professionals and are knowledgeable about their illness, spend less time in the hospital.
The Goals of the Pulmonary Case Manager Program are to:
- Improve the quality of your life
- Increase your understanding of your lung problem.
- Intervene promptly for lung related concerns to prevent emergency visits and hospital stays.
In the Pulmonary Case Management Program we will:
- Talk with you about your breathing problem and what can be done to help you feel better.
- Review the medications you are taking and explain why each is needed. We also will discuss ways to obtain these medications if there is a financial concern.
- Same day appointments are available if you need to see us immediately for your breathing problem. Also, we are available by phone twenty-four hours a day.

- Why does my doctor want me enrolled in the Pulmonary Case Management Program?
Your doctor believes you will benefit from the close monitoring and extensive education you will receive from the program. Cases management adds to the care you already receive from your doctor.
- What will happen during the visit?
The nurse will weigh you, take your blood pressure and listen to your heart and lungs. She or he will talk to you about your condition and review your medications. Blood work or other diagnostic tests may be done at this time. She or he will listen to and address concerns.
- What is the difference between the program and a doctor’s office visit?
Frequently, people with new lung problems or those with complicating conditions require special attention. We will help you through your illness and improve your quality of life.
