Memorial Deep Vein Thrombosis (DVT) Clinic

Deep Vein ThrombosisThe Deep Vein Thrombosis (DVT) Clinic facilitates the appropriate management of patients with blood clots on an outpatient basis with the approval of their primary care physician. As the referring physician’s agent, the staff of the DVT Clinic will coordinate most aspects of anticoagulation therapy (e.g. order appropriate laboratory tests, adjust anticoagulation medications, schedule return appointments, etc.) Anticoagulation therapy will be managed collaboratively with the referring physician. Patients will be monitored until their clotting factors are within a safe therapeutic range then returned to the care of their primary care physician. This program, supervised by a Medical Director, John Ball, M.D., is staffed with two nurse practitioners and one registered nurse.

Goals:

  • Initiate anticoagulation therapy and stabilize blood-clotting factors
  • Assess patient’s health status as related to anticoagulation therapy and monitor response to anticoagulation medications
  • Make dose adjustments based on laboratory values and clinical assessment to maintain patient within target range
  • Routinely monitor for adverse reactions, compliance, drug-food, and drug-drug interactions
  • Facilitate appropriate management of anticoagulation-related complications
  • Provide education and consultation to patients, caregivers and providers
  • Provide quality satisfying anticoagulation therapy to patients and providers
  • Improve quality by providing resources and information to patients
